If you see a notification saying ‘you need permission to access this form’ it means you are logged on with Gmail or another Google+ account
YOU MUST ENSURE YOU ARE NOT LOGGED INTO ANY GMAIL ACCOUNT OR GOOGLE+. If so, you need to log out of these.
You will be asked to login to your DoE account (this is the login you use to access the internet at school) e.g. firstname.lastname@education.nsw.gov.au – make sure you are using your account or your choice will be recorded under someone else’s name.
You must now complete the form answering ALL questions.
Please make sure you choose a summer and a winter sport.
Please also ensure you select the correct gendered sport i.e. Girls Mini-Soccer or Boys mini-soccer etc.
Once completed you must click submit.
An email containing your choices will be sent to you.
If you choose a sport with a fee attached to it, you are agreeing to pay this fee on request.
If the fee says ‘per week’ you will need to bring that money to Wednesday sport each week or you will not be able to participate in that sport.
